Cristo Rey Family Health Center (CRFHC) sees patients of all ages, from newborn through end of life. We offer a variety of primary care services, including physical exams and well child visits, vaccines, telehealth services, sports physicals, point of care testing, management of chronic illness and more. As a Catholic organization, we provide healthcare that aligns with the Ethical and Religious Directives for Catholic Healthcare. While we are a Catholic organization, we welcome patients from all religious backgrounds.

Our Care Model

At Cristo Rey Family Health Center, we see that our patients experience many barriers to accessing proper healthcare. The population we serve experiences an increase in comorbidities and negative health outcomes as a result of delayed access to care. To effectively care for our patients’ needs, we offer no less than 30 minute appointments at each visit.   

Additionally, we acknowledge that continuity of care improves patient outcomes. As a result, we assign each patient in our practice a primary care provider who serves to treat the patient for all non-acute needs. Establishing a trusting patient-provider relationship encourages comfort and transparency for the patient so they can receive the best treatment possible. Our established patients experiencing acute-care needs, such as a cold or stomach bug, can be seen by any provider during our reserved ‘same-day’ appointments, which are blocked on each providers’ schedule to accommodate patients experiencing such illness. 

Our location allows us to connect patients to other vital resources on our campus, including our Cristo Rey Counseling Center and food access programs, reinforcing our commitment to holistic health and community well-being.

Our Primary Care Services

As a family health center, we provide primary care services to families and individuals of all ages. Our scope of services includes the following: 

Acute Concern Treatment

In-person and telehealth appointments are available

Adult Annual Physical Examinations

Including PAP smears, cervical and breast exams, and referrals for lab testing and other preventative health measures such as mammograms, colon cancer screenings, and more

Chronic Condition Management

Such as diabetes, hypertension, cardiovascular diseases, chronic viral illnesses, and more

Clinical Support Visits

Provided by our Medical Assistants for blood pressure checks, pregnancy testing, intradermal Tuberculosis testing, vaccine administration, and more

Substance Use Disorder Treatment and Counseling

Provided on-site by our Cristo Rey Counseling Center

Mental Health Care

Provided by our Psychiatric Mental Health Nurse Practitioner

Well Child Visits

Including all CDC recommended vaccines, lead, and hemoglobin testing

Prescription Assistance Program

Helps patients check eligibility and coordinate between their insurance and the pharmaceutical company to receive discounts on prescription medication costs

New Patient Registration

We are happy to establish care with new patients!  Please call 517-371-1700 to begin the new patient registration process.  Please note: we often have a new patient waitlist, so there may be a waiting period before we are able to see you for a new patient appointment. 

Please arrive 30 minutes before your scheduled appointment so all paperwork and registration can be completed. 

Can I see any provider within the health center?

In order to provide the best patient care, we schedule all preventative care, medication refills, and non-urgent visits with the primary care provider you’ve been assigned.  If you have acute concerns, such as a short-term illness, and need an appointment sooner, we are happy to schedule you with whichever provider has availability on their schedule.  

We currently do not allow patients to switch primary care providers within the clinic without a consultation with both their provider and the office manager.

For patients without insurance, we offer affordable options to ensure access to our services. Our Sliding Fee Discount Program provides reduced rates based on income and family size. What does it mean to be a Catholic practice?

We believe that every patient we serve is made in the image and likeness of God, and must be treated with dignity and respect as God calls us to do. We treat every patient in this manner, regardless of their own religious beliefs. The ethical decisions we make surrounding the care we provide are also derived from our belief in the dignity of human life.

What is the benefit of receiving care from a Catholic practice?

If you are Catholic, you can feel comfortable knowing that we will provide care in alignment with the values of our faith. If you are not Catholic, you will still be treated with the same dignity and respect that everyone deserves. Will I be pressured to become Catholic or religious?

No. You do not have to be Catholic or religious to receive any CCIEC service, and no program will pressure you to beome Catholic or religious. If you have questions about how your religious beliefs can be integrated into your care, please speak with your provider.

How does the CRFHC handle issues like abortion and birth control?

As a Catholic practice, all of our ethical decisions reflect the Ethical and Religious Directives of the Catholic Church. Guidance on issues like birth control and abortion as described in the Directives are derived from our belief that these actions are harmful towards your dignity. We are happy to discuss any concerns in detail if they arise.

Diversity at CRFHC

We serve a diverse population at CRFHC. Just under half of our patients speak Spanish and identify as Hispanic. Therefore, we make it a priority to have bilingual staff on our team who can bridge the language gap for our patients and break down just one more barrier to receiving healthcare. We are proud to say that roughly half of our staff speak fluent Spanish, and a handful more are also fluent in Vietnamese, Arabic, and Portuguese.

St. Vincent Campus Expansion

In 2025, our services at CRFHC expanded to a second location at our St. Vincent Campus! We added five additional exam rooms to our practice, offering the same health services as the Cristo Rey Campus location. This expansion helps meet the growing need for quality, affordable health care in our community.
